I ended up giving up in Mineral Springs. Mesmer boss rarely spawns.
One would think that Ineptitude would not be so difficult to find....

keep in mind if you make the complete loop in that map there are either 5 or 6 (cant remember which) avicara bosses out there.... so if you didnt do the WHOLE map, chances are good he spawned and you just didnt find him.

There ARE 6 spawn points in Mineral Springs, so each boss has to be there...

Does Ineptitude end after the foe attacks like clumsiness does? Or does it keep doing damage every time foe attacks (if they are stupid enough to keep attacking) for the hex's duration?

just like Clumsiness, ends after they attack, and blinds them.

if this is the case they need to fix the skill description
...
Ineptitude (Elite)
Elite Hex. For (Min: 4 - Max: 10) seconds, if target foe attacks, that foe takes (Min: 10 - Max: 82) damage and becomes Blinded for 10 seconds.
where does it say that it will end on an attack:
Clumsiness
Hex. For (Min: 4 - Max: 8) seconds, the next time target foe attempts to attack, the attack is interrupted and target foe suffers (Min: 10 - Max: 92) damage.
clumsiness states that its only the next time the foe attacks, something needs to be fixed.
